The Transforming Episode Accountability Model (TEAM) is a new Medicare initiative launching in 2026 that holds hospitals accountable for care during and after surgery. By coordinating recovery more effectively, it aims to improve patient outcomes, reduce readmissions, and lower healthcare costs.

TEAM shifts responsibility from just the procedure to the full episode of care, including 30 days of recovery and outcomes.
